Primary Care Partnerships
Primary Care Partnerships (PCPs) are voluntary alliances of health and community support service providers across different geographical catchments of Victoria. The Primary Care Partnership strategy is a Victorian Government initiative that provides funding across the state for PCPs to strengthen relationships across sectors in order to maximise health and wellbeing outcomes, promote health equity and avoid unnecessary hospital presentations and admissions through:
- Partnership Development
- Integrated Health Promotion
- Service Coordination
- Integrated Chronic Disease Management
Members represent a broad range of services including local government, hospitals, community health, women’s health, community nursing, palliative care, mental health, CALD, Aboriginal and Torres Strait Islander, family and children’s services, disability and others.
